The Oak Park Office
NOTE: Our Oak Park office will be moving effective 2/5/2018. Our new location will be in River Forest, IL.
We are sharing offices with Father's House Christian Church in a seven story white office building on the corner of Lake Street and Forest Avenue.
Note to our clients:
- Free parking garage is on northeast corner of Lake Street and Forest Avenue. Enter from Forest Avenue.
- Entrance to lobby/elevator is on east side of building. The lobby directory and the sign outside Suite 107 will note Father's House Christian Church.
- This location is handicapped accessible.
- If possible, please print and fill out our new client forms and bring them and your insurance card (if applicable) to your first appointment.
