Parking lot is off of Lake Street. Clients enter beige door (by the big green windows) with CCCC sign on side of church building. Please ring doorbell and therapist will come greet you.
NOTE: Handicapped accessible entrance is off of 9th street. Enter main entrance door in back of church building. Please ring doorbell and therapist will come greet you.
If possible, please print and fill out our New Client Packet and bring them and your insurance card (if applicable) to your first appointment.
